- What to expect
When you arrive for your initial visit, you will be asked to complete new patient paperwork. These documents include a brief medical history, a consent form and insurance/billing documentation. Please remember to bring your prescription for physical therapy (provider referral), insurance card(s) or coupons and any other diagnostic reports relating to your injury.
Once your paperwork has been completed you will meet with your Physical Therapist. Your therapist will review your medical history and provider referral, perform an initial evaluation, discuss your therapy goals with you, design a treatment schedule and home exercise plan to get you back to your active lifestyle quickly and safely and, of course, answer all your questions. Additional appointments will be scheduled at this visit.
- Do I need a referral?
Although you are able to receive physical or massage therapy services without a prescription from your physician, please keep in mind that your insurance carrier may not pay for physical or massage therapy services without one. A physician referral is required to receive occupational therapy.
- Insurances we accept
We are a preferred provider for most insurance plans, including auto insurances if your injury is due to a motor vehicle accident. Please reference our list of accepted insurance plans, or contact one of our billing representatives by calling 1-866-687-3349
- What to wear
Please wear comfortable/athletic type clothing and shoes so that you can move with ease. You may also be notified ahead of your visit to bring clothing appropriate to use in one of our therapy pools. If you are unable to bring gym clothing for any reason, we are happy to loan you a clean set.
- How long is the visit?
Each appointment is 40-60 minutes in length, although your therapist may have you arrive early to begin a warm-up routine or to do some stretching.
Please have paperwork filled out or arrive early Please download, print and fill out the appropriate form found on the right side of your screen (in the dark blue box). If you prefer to fill out your forms at the clinic, please arrive 15 minutes early to allow enough time with your therapist.
- Once I am done with physical therapy what can I do to stay fit?
Your therapist will design a home exercise program tailored to your specific injury. This program will be modified as your rehabilitation progresses and you will be given instructions to continue certain exercises when your treatment is complete. Your therapist will also discuss activities and other modifications that with keep you moving safely. We also offer a discounted gym pass that allows you to continue to use our equipment and stay in touch with your therapist.
